Brain Death and Organ Transplantation


Abstract
The acceptance of brain death has brought numerous interest as well as confusion. The reason certainly lies in the argument for various transplantation. Because of this, sociocultural, ethical, philosophical, as well as legal and medical aspect
on
brain
death argument has been discussed.
Medically, brain death deals with situation that has normal circulatory system besides the brain which was damaged irreversibly. And because there is no spontaneous respiration, artificial respiratory support has to be done. However, even
artificial
respiratory support the body tissue disturbs on a given time(as rapidly as 30minutes and as long as 2weeks) such as necrosis of functional loss.
In Europe and America, kidney, liver, heart, lung and pancreas transplantation and so as to multiple transplantation of heart-lung, heart-lung-liver, liver-small intestine-pancreas, liver-kidney, suprising development has been occuring. In
regarding
live transplantation, reduced-size liver transplantation can also be done, And even various prognosis of transplantation has very much improved so the success rate is approximately above 90% and approximately 70% of patients become so well that
they can
have a normal healthy life.
In our country, the most active field of organ transplantation that is renal transplantation. However, other organ transplantation are in informal situation some instances of liver transplantation and pancreas transplantation have been done by
the
help
of donation of organs by organ doners. There is, of course, legal problems regarding organ transplantation because of unapproved brain death in our society. The legality in our society permits the donation by living related organ doners of kindey
but
other such as liver, heart, lung, pancreas transplantation can not be done.
Our clinical medical field does not have the approval on brain death, and therefore various organ transplantations have not been performed. This causes many realistic problems.
Firstly, we can not afford the best or elective treatment to the terminal patients.
Secondly, scientific development becomes delayed, so our medical field has been left behind.
Thirdly, nation's medical aid or burden of expense increase tremendously.
Fourthly, terminal patient's organ transplantation is performd in common other country for treatment so apt span of time for treatment does not happen or excessive expenses can be brought.
Fifthly, living organ doners, unrelated organ transplantation can lead to sale or illegal actions.
Sixthly, liver, heart, lung and pancreas transplantation have to be done only with brain death, which is crucial for human body and the people who get involved in this cause medical, ethical and legal problems.
Seventhly, after brain death harvested organ is not properly stored and cared for and therefore, it can lead to primary nonfunctioning graft of transplanted organ and other medical problems.
Eightly, national misunderstanding on brain death leads to confusion on organ donation.
Even if brain death approval becomes legal, after brain death without holy spirit of organ donation, organ transplantation can not achived. Futhermore, judgement on brain death and organ donation, preparation of donated organ, and process leading
to
organ transplantation becomes a bigger problem and legal judgment of this process has to be ready.
Brain death has to be approved legally and after brain death, legal basis of organ donation should prepared, so various organ transplantation becomes active.
It is difficult to perform organ transplantation only by he transplant surgeon's standard technical knowledge. Construction of generalized management system is needed for organ harvesting of brain death doner, test for tissul or organ matching,
adequate sharing of the organ, transportation, transplantation.
Transplantation surgeon as well as physians has related to transplantation absolutely can not participate in the judgement of brain death.
Organ transplantation does not occur because of brain death. After brain death, only holy spirit of organ donation, many lives that can be saved by various organ transplantation could persisted.
